Personal meeting with the Violation Inspector

PUBLIC – when the reporter agrees to full disclosure of their identity to both those involved in investigating the report and to third parties.
CONFIDENTIAL – when the reporter’s data is protected from unauthorized access, in particular by keeping the reporter’s data confidential.
ANONYMOUS – not allowing the reporter to be identified in any way.

Details of the reporting person (name and surname, place of work, position), including contact details (excluding anonymous reports)),
Date and place of the report,
Details of the person(s) who committed the violations that are the subject of the report (name and surname, place of work, position)
Description of the violations, including the circumstances in which they occured and the dates,
Description of the actual and potential consequesnces of these violations,
Actions taken by the reporting person to eliminate the violations or their effects (including previous reports, e.g., to superiors), if any,
Evidence collected by the reporting person to confirm the facts, e.g., documents, witnesses, if any.
